practices for every service.

Although I make every effort to provide a positive experience, individual responses to holistic wellness services may vary. As with any wellness service, there is a possibility of mild discomfort, sensitivity, or other
unintended reactions. By choosing to participate in these services, you acknowledge and accept these potential risks.

The KYSS Experience and its practitioners are not licensed physicians or medical providers. Offered services are intended to promote relaxation and support overall wellness and are not intended to diagnose, treat, cure, or prevent any disease or medical condition. Nothing offered during your visit should be interpreted as medical advice or used as a substitute for evaluation, diagnosis, or treatment by a qualified healthcare professional.

If you are pregnant, nursing, have a medical condition, have recently undergone surgery, are taking prescription medications, or have any concerns about whether a service is appropriate for you, please consult your healthcare provider before scheduling or receiving services.
 

By booking and receiving services from The KYSS Experience, you acknowledge that you have read and understood this notice, have disclosed all relevant health information to the best of your knowledge, and
voluntarily assume responsibility for your decision to participate.

Photo & Video Release Authorization

Thank you for allowing The KYSS Experience to share your wellness journey with others. From time to time, we may take photographs, videos, or audio recordings during services, events, or while you are visiting our spa. Your privacy and comfort are important to us. No photos or videos will be taken without your
knowledge, and signing this form is completely voluntary.

I voluntarily grant permission to The KYSS Experience | Kesi's Yoni Steam Spa LLC, its owner, employees, contractors, and authorized representatives to photograph, videotape, and/or record me and to use my image, likeness, voice, and/or testimonial for lawful business and promotional purposes.

I understand that these materials may be used in, but are not limited to:
☐ Social Media (Facebook, Instagram, TikTok, YouTube, etc.)
☐ Website
☐ Printed Marketing Materials
☐ Brochures and Flyers
☐ Advertisements
☐ Educational or Promotional Presentations
☐ Other Business-Related Marketing Materials

I understand that these images, videos, or recordings may be edited for formatting, clarity, or length but will not be altered in a misleading or defamatory manner.

I acknowledge that I will not receive financial compensation for the use of these materials.

I understand that once photographs or videos are published online or shared on social media, The KYSS Experience cannot control how they may be copied, shared, or redistributed by third parties.

Revocation
I understand that I may withdraw this authorization at any time by providing written notice to The KYSS Experience. I understand that revocation will apply only to future use and cannot require the removal of materials that have already been published, printed, or distributed before my request was received.

Release
I release and hold harmless The KYSS Experience | Kesi's Yoni Steam Spa LLC, its owner, employees, contractors, successors, and assigns from any claims, demands, or liability arising from the authorized use of my photographs, videos, recordings, or testimonials, except as prohibited by applicable law.
